Member Application
To qualify for membership, you must be an OB/GYN surgeon.
To apply for membership, the following documents are required:
- Operative report of the hysterectomy attesting that the procedure was
performed without resorting to laparotomy
- Pathology report attesting that the uterine specimen removed weighs one
kilogram or more
- Complete and current curriculum vitae
- Completed Personal Information Form -
